Application materials should be received no later than January 5, 2015 at 11:59 p.m. Upon evaluation of application materials, qualified applicants will be offered an on-site visit and interview with the residency team. Interviews take place in late January and February. Successful candidates will be expected to graduate with a Doctor of Pharmacy degree, have an active BLS certification and obtain Indiana licensure by August 1. The application process will include a formal employment application to be completed upon successful matching with our program.
