Our Quality Report Cards
Community Health Network strives to keep patients and visitors informed and involved in their health care decisions. We believe that, as consumers of health care, members of the community have a right to know about our hospitals’ quality of care and how it compares to national performance goals. That’s why we publish our quality report cards online.
You may view our performance by condition or by hospital.

By Hospital
We believe that informed patients make better choices about their providers and are able to participate better in their own care.
The information here highlights how Community hospitals and the network as a whole perform on a variety of key quality indicators. It is important to note that performance in each category varies over time and among hospitals, health care teams, and physicians. This is why we report average data over a longer time period, usually 12 months at a time. These averages help provide a guide for future performance, but are not the only means by which you should select a health care provider.
