Community can provide your sports physicals or sports injury screenings (concussion, heart health) for student athletes.
Sports physicals are available through Community Physician Network primary care physicians, sports medicine care physicians and MedCheck walk-in care throughout the year. At the end of every school year and through the summer, sports physical clinics are offered at our area partner schools.

Community Health Network participates in the Indiana Sports Concussion Network in support of concussion education and management using the ImPACT concussion evaluation system. $25 student athlete echocardiogram
This cardiovascular screening is designed to evaluate the potential risk of sudden death from a cardiac related abnormality such as hypertrophic cardiomyopathy (also known as IHSS/HC/HOCM) and long QT syndrome (LQTS). Eligible students must be at least 10 years of age. Students under 18 years must have a parent or legal guardian present to sign the consent for testing. *It is important to understand that this special screening is limited in nature and does not evaluate for all potential cardiovascular conditions that could exist now or in the future, including valvular abnormalities or coronary artery disease. This examination should not replace a complete cardiovascular examination if indicated by symptoms, murmur or your physician. If the screening results are abnormal you should immediately seek follow-up care with your personal physician. This screen is limited to only cardiovascular care and does not evaluate for non-cardiovascular issues.
